Free templates

Free printable templates for the front door of a UK site

Sign in sheets, a visitor health questionnaire, a contractor induction record, two permits and a fire drill record. Each one prints from its own page as a clean A4 sheet, with the privacy wording or the safety precautions already on it. None of them is behind a form, and we would rather you used them than a blank notebook.

What is here

6 templates, all free, all printable

Each page has the sheet itself, how to use it so it stays lawful and useful, the sources it is built on, and an honest list of when paper stops being enough.

Visitor and contractor sign in sheets

A visitor sheet, a contractor sheet and a fire roll call register, with the UK GDPR privacy line already written and the habits that keep a paper register lawful.

Three A4 sheets

Visitor health questionnaire for food sites

The illness questions food and packaging sites ask at the door, the hygiene rules ticked one by one, and the host’s decision, with the privacy line for health data.

One A4 sheet, also as PDF and Word

Contractor site induction record

The checks before work starts, the induction topics ticked one by one with brackets for your own site rules, and a signed declaration from the contractor and the person who inducted them.

One A4 sheet

Permit to work

A general permit built on the essential elements HSE’s guidance lists: hazards, isolations, precautions, authorisation, acceptance, hand back and cancellation.

One A4 sheet

Hot work permit

The fire precautions pre-printed as a checklist, with the fire watch periods insurers expect: during the work, sixty minutes after, and checks to two hours.

One A4 sheet

Fire drill record

Timings, the roll call count including visitors and contractors, what the observers saw, and the actions that came out of it, on the sheet an enforcing officer asks to see.

One A4 sheet

Free, and not paper

Rather go digital for nothing?

A QR code at reception, Microsoft Forms and a spreadsheet, built in about an hour from things your business already pays for. Eight steps checked against Microsoft’s own documentation, a Teams alert for the host, and an honest list of where it stops.

How they work

Five rules every template here follows

  • No form, no email address. You are looking for a sheet, not a sales call. Every template prints straight from its page, and nothing is collected when you do.
  • The page is the template. Pressing print hides everything except the sheet. What prints is what is on the page today, which is why most of these are not PDFs: a file in a downloads folder cannot be corrected.
  • Brackets where the content is yours. Your site name, your rules, your retention period, your assembly point. A free template cannot know them, and one that pretended to would be worse than a blank sheet.
  • Sources on every page. Where a sheet carries a figure, the fire watch periods on the hot work permit for instance, the page links to the HSE, GOV.UK or industry document it came from so you can check it.
  • Starting points, not advice. We are not safety consultants, fire risk assessors or lawyers. Each sheet is somewhere to begin, and each page says what it does not do.

Being straight with you

Why a kiosk company gives away paper

Because somebody searching for a sign in sheet is not buying a kiosk this year, and a well run paper system is a perfectly good answer for a lot of sites. If the sheet came from here, and it was honest about where paper runs out, this is where you will look when it does.

It usually runs out in one of three places: somebody outside the business asks you to prove what a sheet says, the alarm goes and the list is inside the building, or the folders outlive the retention period written on them.

When you are ready

If the sheets have stopped being enough

Fifteen minutes on the phone. Tell us who has started asking you to prove things and how many doors people come through, and we will tell you straight whether a kiosk is worth your time yet.

Book a 15 minute call

If the paper is still doing the job, keep using it. We will say so on the call.
